RESPECTING AND INVOLVING CLIENTS
We have a person-centered approach to care and believe that all clients have the right to participate in their own
care as far as possible; an advocate will support this when needed. Focusing on the needs of the indiviual means adhering to our Diversity Policy, keeping clients informed about the risks and benefits of their care, listening to their
views and encouraging them to manage and understand their care.
Our concern for the dignity and privacy of the clients is demonstrated by the individual way we treat each one in
relation to their physical, emotional, social and educational needs. We respect clients’ rights to
participate in organisational decisions that effect the day to day running of their home and support them in
communicating their views.
If a client does not have capacity we will work with relatives and /or an advocate to support them through the
following: the process of being informed about their care, discussing the risks and benefits, understanding decisions
and the possibility of changing them. In this way we can be confident that the human rights of every individual are
respected.
Staff are trained in the Mental Capacity Act and The Deprivation of Liberty Safeguards. Each client has MCA Assessments and a DoLS assessment in their Care Folder, which is reviewed 6 monthly, or earlier if needed. Staff sign and
date these papers to ensure compliance with Government guidelines. Where a client has capacity to make certain
decisions they will be at the centre of decision-making processes and be able
to give informed consent to their care through their Keyworker, Care Plan and Care Review system.
CARE REVIEW
A Care Review evaluates the client’s progress and considers the goals set in their original needs assessment or
subsequent Care Plan. The Review considers the continuing suitability of a placement, discusses future plans, and
develops multi-disciplinary strategies to addressing specific issues and needs.
A Care Review is held annually for all clients, although a client or their relative may request one at any time.
They are multi-disciplinary with the attendance of the client, family and friends, a member of the Management
Team, Advocate or Best Interest Assessor (when necessary), Care Manager, Keyworker, other external professionals
(if necessary). Clients who have capacity will be involved in their Care Review and are involved at all stages as
far as they are able. Reviews are chaired where possible by the client or if not, the Care Manager or ECC Manager.
An agreed written record is kept in the client’s Care folder.

INDIVIDUAL CARE PLANS REFLECT:
- The consent status of the client,
e.g. does the person have ‘capacity’
to make decisions when they are
needed
- The needs, aspirations and goals of
the client and a transition to more
independence where possible
- The ways in which The Conifers will help
the client meet their needs and achieve
their goals
- A balanced and structured approach to risk
taking and Risk Management
- Positive strategies and approaches to
managing and changing behaviour that
challenges
- Activities, employment, education and leisure
opportunities offered to the client
- Nutritional requirements, likes and dislikes
- Any necessary infringement of rights or
restrictions of choice that are agreed with
the client, thier advocate, family or multi
disciplinary professional team
e.g. Care Manager, Community Nurse and
or a Doctor
